Request another adult’s information
An individual may ask a third party to make an information request on their behalf. This could be a relative, friend or solicitor.
To make a request on another adult's behalf, you must prove that you have the authority to make this request. Evidence of your authority might include:
- Power of Attorney
- Court of Protection Order
- written Consent of the Data Subject (DOC, 55.79 KB)
- a best interests’ decision by a professional working with the individual - this might be made after a mental capacity assessment
We are not required to process your request if sufficient evidence is not provided.
If the information is sensitive we may contact the individual to discuss it first. If they agree, we may send the response to them rather than to the third party requester. They can then choose to share the information with the third party.
To request information about another adult, you will need to provide:
- your full name
- your address
- your telephone number and / or email address
- proof of your identity, such as a valid passport, driving license or full birth certificate
- proof of your authority to make a request on their behalf
- the full name of the adult you are making the request for
- proof of identity for the adult you are making the request for, such as a valid passport, driving license or birth certificate
